Alignment Health is working with individual doctors to create subscription health plans for families with Chronic patients. Through our plan, we are flipping incentives for the doctors, such that doctors end up benefiting more financially if their patients remain healthy, vs when they end up needing more care (as is the situation with current healthcare system).
